[Plugin: Ajax Contact Module] great plug-in / question

this plug in is great. i’ve styled it an implimented it hereand it is working great… except that it is not showing up on a few of my pages… for example: it shows up on my video page… but not on my blog page. and those pages are identical accept for the queries. do you have any ideas on why this may be ? if i can just fix that – i’ll be using this plugin everywhere. thanks.
