    We have tried to install a demo page with wordpress 3.9 and the latest version of DW Question Answer ( http://prntscr.com/3mmnff ), we didn’t meet your problem, can you double check or try to re-install plugin. If you still meet this issue please go to designwall.com/question, submit a private question and give us an admin account for furtherchecking.

    Thanks for getting back to me!

    This is on a localhost development site, so I can’t give you access. But the problem is intermittent so, especially since you can’t reproduce it, I think this is probably a plugin conflict. I have tried disabling all other plugins — and the most likely culprit seems to be Query Monitor itself!

    Actually, I have finally managed to identify the culprit as MainWP.

    kts195, if you provide us some details of the issue you found we would be happy to take a look.

    There aren’t many details: running DW Question & Answer and MainWP Dashboard plugins on the same installation produces the above error in Query Monitor. I am running WP 3.9.1.
