Permalink issues

once the post is copied usually my plugin has no other effect on WordPress standard behaviour, which is that you can’t have two identical permalinks, obviously.
I see that http://riverstonewellnesscollaborative.com/phillip-bass/ exists, so WordPress adds a -2 to make sure the new permalink is different.

What’s strange is that http://riverstonewellnesscollaborative.com/phillip-bass/ is actually an attachment page, but usually it should be something like http://wp.example.net/path_to_post/post_name/attachment_name. I suppose there is something that’s changing attachments permalinks?
A quicker solution to the problem without touching the permalink structure would be to go in the Media Library and edit the permalink of the image.
