Password Reset Not Working

When a visitor to my website clicks on the lost password link, they are sent the password reset email. When they click the link in the email to reset the password, it redirects them to http://www.[redacted].com/wp-login.php?action=rp instead of resetting the password. Can you please advise how this can be fixed?
