Participants Database Add form

    I have an extended list of fields in my participant database. I have added the Add Participant link to my New User Setup page. When I preview the page on the computer with which I am working with WordPress, I see the complete form as designed. However, when I go to the site using a different computer, I get the short User/Password login form. I went back and checked if my page was publshed, and in settings checked the box allowing anyone to register on my site, and updated that form as well.

    No help! I want my site to be usable by anyone without requiring pre-signup or permission. How can I get this done. The site is FindStageResources.com.
