It looks like we don’t have proper support for your theme. We are working on making the manage page a shortcode so you can put it on any page. I would recommend for now turning of the auto theme include. This will make the manage page look like this:

Probably not ideal but it will be functional. Hopefully the shortcode will be ready in the next major release in a few weeks.
