opt-in newsletter

Need some help / recommendations on a plugin that I can add to my WP website as a widget to say “OPT-IN and GET a FREE XXX” where XXX is an email message (HTML Format) sent to their email address after they enter their email address and click “Submit” button. I also want to capture their email address in a DB for me to use as future reference.

I currently use a plugin called “Mail Subscribe List” which does only capturing of the email address in a DB on WP.
