Old version forms

Dear Programmer,
I am using Caldera Form during 1 year, but I updated from 4.8.5 WordPress version to 4.9.4, and Caldera form from 1.5.0.5 to 1.5.9.1.
But now the form is not working. I can not go to the next page, as if the form is not recording the data.
I can create a new form and it is working, but my old form is not working.

in addition information, each time I download the old form, there is a message indicating script problem:

In fact the problem is not coming from my olf form, but from the new version of caldera forms plugin.
There is a problem with the “Toggle Switch” field type.
The form is working without “Toggle Switch” but not working with it.
Please make a try and let me know what to do.

There is also a problem with the “Checkbox” field type. I can not go to the next page. But the “Dropdown select” field type is working well.

There are two good ways to access older versions of any plugin from WordPress.org, including ours.

The first is to download a ZIP file using the “Previous Versions” section of the “Advanced” section of the plugin page. This is hard to find in the new design, but it is there. https://wordpress.org/plugins/caldera-forms/advanced/

There is a great plugin called WP Rollback that lets you change which version of a plugin you have installed. https://wordpress.org/plugins/wp-rollback/
