I’ve visited the page in question but it seems that the form is not there currently, I only got an empty page. I wouldn’t be able to check form configuration/behavior anyway because e-mail addresses are not included anywhere in page/form source code.
I’d like to test that form so could you, please, export it and share with me? You can export any form from Forminator by using “export” option using the drop-down menu under the “gear” icon on forms list.
It would be best to either share it via pastebin.com service or use exported file and put it on some file storage such as your Dropbox, Google Drive or similar and share a link with me here.
I or one of my colleagues could then import such form to a test site and check its configuration and behavior so we could find out what’s happening and how to fix it.

On the “blank page” there are 29 vertical TABS – each with a separate form. The list of tabs start with Welcome – the blank page. Below Welcome the list of tabs continue… Just please click on “PRINTING” (6th from the bottom of the list) – that will show the form in question…
IPlease let me know if you can’t see waht I am refering to above…

Thanks for response and explanation but I’m afraid it’s not working for me anyway. This is exactly what I tried and I’m still getting blank page, no matter which tab I’ll select:

I did notice, however, that I’m also getting a long set of “Mixed content” errors in browser where some CSS files where blocked by browser due to being servered over insecure HTTP connection to the HTTPS enabled site.
Are you sure that site’s properly configured for SSL? Please take a look at “Site Adreess (URL)” and “WordPress Address (URL)” options on “Settings -> General” page in site’s back-end. They should both start with “https://” prefix if the site is meant to be loaded over SSL.
You might also need to clear all caches on site/server and, if you are using any builder-type theme and/or any asset optimization plugins – to regenerate CSS assets too.
That said, I’d still much prefer to take a look at the form itself on a test site so if it would be possible to provide export of the form, it would be really helpful, I believe.

Hi Adam.
In part my stupidity and lack of proper understanding of Forminator features are to blame for that. You can try and view the form “Printing” again now – should be showing. Between the site menu and role visibility rules and some Forminator settings it was not showing to logged-out viewers. I disabled the Forminator settings (allow only logged in users to submit / honeypot) and its showing now…
Will look into the ssl issue…

Apologies for the delayed response here. I checked the form and I see that there was no space in between the emails and comma added to the form. It should ideally be like this email1@email.com, email2@email.com, email1@email.com but initially, it was email1@email.com,email2@email.com,email1@email.com
I made the required change and it worked for me and emails were delivered on my emails. Could you please import the following form, add it to a page and check if that works for you? Here is the export: https://pastebin.com/4PT6JWks

Prathamesh, Thank you for your reply. That did send the mail to the gmail account – but still not the other two (searchzone)… Having the server checked for issues with email to specifically that domain… will keep you posted.
