We need more information to be able to help you. What version of WC you are using? If you disable other plugins except WC and ours you notice any change?

We’ll release an update as soon as possible. As a temporary workaround you can download the previous version of the plugin (version 3.3.0) here: https://wordpress.org/plugins/woocommerce-bulk-order-form/advanced/
The previous versions can be downloaded at the bottom of the page. First remove your current version of the plugin and then install version 3.3.0. Plugin settings will not be affected by this.
Version 3.3.2 will resolve this issue. So when you see an update notification for 3.3.2 you can safely update.
