Hope to fix this soon, but first must release a new website for our organization. i’ll post back when updates have been made.
I would like to confirm that upon activation that it did not create the tables either. After I manually created the tables, it seemed to work fine though the shorcodes aren’t working off the template file (known tinyMCE issue I believe).

Okay, finally able to spend a little time with this issue tonight. i rewrote the install/update functions and think i may have it working. Will test a bit more and likely post an update tomorrow. Thanks for your patience!

Hello! 0.2.1 is available. It worked for me on a fresh WordPress 3.5 install. Database tables were created. I also updated my current version and database tables updated properly. Let me know if you are still encountering issues or whether we can mark this one resolved. Thanks!
I set up the database per dambacher’s suggestion. The new version worked and showed the new version number as well as the test prayer on upgrade so it seems to work now. I haven’t tested it with a fresh install.
