Not generating new PDF thumbnails?

towards the middle of the page, we are displaying our document gallery, it’s been working fine, but after adding a new document, it’s not generating the thumbnail.. I did try disabling all other plugins and then removed/re-added the PDF file to the document gallery. after giving it a few minutes to render the thumbnail, and not seeing it, not sure what direction to take.
