I’m sorry to hear about your problem! When you say the links in the plugin show “page not found”, do you mean that you’ve successfully installed the plugin and when you navigate to this URL (our plugin’s starting page in admin) you get a “page not found”?

I see you also mention that you’re using permanent links. I’m not sure what this means, but we have tested this with URLs being rewritten with pages and posts. Here’s a working instance of it on a rewritten URL on our own site:

Also, can you provide which version of wordpress your running and, if you’re comfortable sharing, the URL of your site?

I’m having the same issue, but only with the Instructions and Dashboard links. Using WP 3.9

Add New and All Trees are working correctly.
