No radio buttons??

Hi
since we havent been able to set up our form with Contact Form 7 we are looking now for an alternative to set up something like this:

Questions:
-Can this be done with your free version?
-I couldnt find any radiobuttons in your freeversion. Am i wrong or was i just not able to find them?
– how can checkboxes made in a way that only one box is allowed to be checked?
