Need plugin to create training registration page

Hi

I wonder of someone could advise me about a suitable plugin. I’m trying to build a form with conditional logic to use as a training class registration system. I’d like the one of the fields in the form to display a list the categories of classes, and the two subsequent fields to allow the user to select a start and end date. After populating these fields, the user would click a submit button. This will run a query that would populate another field with a list of all classes in the selected category and within the date range. The user should be able to make multiple selections from the class list. A final submit button should fire off an email with this info (and a few other bits of user information) to a static email address.

Does anyone know of a plugin that could accomplish this?
