Need help with comment parameters on WordPress site

    I have a WordPress site and want to add a name and email address field to my comments area at the bottom of my posts and make those fields required. I checked the box on the discussion settings labeled “Comment author must fill out name and e-mail ” but those two fields do not show. Please tell me how to make the two fields show.

    My subdomain is test1234.laughandheal.com.

    It might be a plugin conflict as I can see the email field but not the name field. When I tried to leave a comment it showed error and said I must enter my name. Try disabling all plugins and test.
