Need help finding this widget (a ratings type widget)

Fairly new to this but am hoping for some help. I am looking for a widget that operates in a similar way to what I have included below. I have searched everywhere and done searches on “ratings” widgets, etc. Maybe I am just not putting in the right thing? Any help you can provide in helping me to find something like this would be greatly appreciated.
