Multiple issues with page analysis

I just installed seo yoast and am having multiple problems with the page analysis section. Its installed on an existing wordpress site and I have several red and yellow indicators that won’t change no mater what I do to the text. What am I missing or what am I doing wrong? The first issue is it says my keyword phrase is not appear in the page but it does several times. The key words also don’t show up in the first paragraph or in the title but they do. It states that a meta description has been specified but is not in the target keyword phrase but they are there. It reports 0% keyword density but the words are there. And the last one is that images on this page do not have alt tags. I only have one picture on the page and it does have an alt tag.
