Do you mean images on the server in the uploads folder? or multiple images as in set as a featured image, and then also inserted into the post content multiple times?
Can you refer me to the topic you mention as well? It’ll help me get a better frame of mind of what’s going on.

Only way I can think of, off the top of my head, for why that would happen is if it finds need to import/sideload into the media library multiple times. It’s set to return early if there’s already a featured image set. I’m now curious if we are naming the file in a predictable way and could then check for an existing media library item based on that.

At the exact moment, yes. I don’t know yet when that media library file checking will get done.
If you would be willing help test some things, I’d appreciate it.
Try out the version from https://github.com/WebDevStudios/Automatic-Featured-Images-from-Videos/archive/master.zip and see if it still occurs.
At the same time, remove the featured image and re-save posts, and see if new copies end up in the library. Not sure how much editing of the post above had been done in possible similar ways.
I’ll be doing similar tests on my end at the same time, but want to have someone else experiencing the issues help confirm as well.
