Multiple choice and written

Hi, i would like to make something like this:

Transport:
Bicycle n.
Car n.
Vans n.

I know how to put a multiple choice but how can i create a multiple choice where people can the write the numbers of the transportations at the same line?

How can i change the form of the box where the people write their name, last name, address, ecc….
