Multiple calculations in one formula

I’m looking to create a form that calculates the following.

A user enters how many people they have in their email database multiplied by an estimated response rate as a percentage. (I’ll instruct them to enter a number between 3 and 10).

Then multiply that by their closing rate percentage (I’ll instruct them to enter a number between 50 and 90%).

Then multiply by an average dollar amount that is their customer’s lifetime value then spit out a number.

I’ve already created all the forms but for the life of me, I can’t figure out how to do the multiplication of each field (and as a percentage).

Can you show me how to set that up using your plugin?
