Multi Choice question and answer.

I am trying to find a plugin that will allow a user to have a choice of words and dependent of the word/s they have chosen will then give them an answer (text format).

e.g.
If we have 2 warehouses one called A and the other called B and have 3 products 1, 2 & 3.

If they entered/chose 2 and the item was being shipped from warehouse B then it would tell them that.

If anyone know of a plugin that would allow me to do this?
