So are you saying when you try to insert media, it is not showing you the latest images that are in the media library? What version of wordpress are you using and what theme?
We use last WP version
We insert directly media on product page (Woocomerce) without problem
When we want insert media on page we going on Library and we have any new pictures
Insert Media / Library don’t show all picture from WP Medias Library

Are you saying you are inserting images using WooCommerce and then you are not seeing them in the library? Have you checked on the WooCommerce forums as well?
The insert image by woocomerce ass wel on the library when we open Library by the WP Menu
The problem is when we create a page and we click on “insert media” the images display are not update and we have any new image?

Do you have this issue when using the default theme? Have you tried disabling plugins to find if one specific plugin is causing the issue?

I have the same problem:
My medialibrary (in dashboard) shows all uploaded pictures, video’s, etcetera. But when i want to add a mediafile to a post or page, the newest uploaded files seem to be missing. Again, they are still in my medialibrary in the wordpressdashboard.
I’m no expert in building websites: http://www.emclogics.com, but i need to fix this issue. Is there anybody that can help me?
What i did:
Deactivate all plugins,
Back to default theme
Hotfix (plugin)
check ftp-server (media is there)
I have the same problem. In the Media Library, all images are there. But ‘Insert Into Post’ only shows about 1/3 of them.
I find all the images in /wp-content/uploads/2014/01 and /wp-content/uploads/2014/02. Why aren’t they on the ‘Insert…’ page?

I had this problem too on a couple of sites after updating to 3.8.1. It seems to be fairly common but not officially acknowledged as a bug as far as I know (which it should be). In my case the meta data for newly uploaded images was getting corrupted. If you search here you’ll find various fixes you can work through.
I also suggest you each post a separate question – the root cause could be different and maybe the issue will get more attention.
