Check wp_posts table and make sure ID column is primary key!
I don’t why but my problem was with primary key in this table, it is dropped!
Same issue with 4.5.2. My Media Library is empty.
I just deactivated Instagram Feed Pro and now I can see my photos in the Media Library.
Clearing browser cache does not fix the issue like it did for Facebook Feed Pro owned by smash balloon.
Updating Instagram Feed Pro to 2.0.4.2 has resolved the issue with WP 4.5.2
Same issue with 4.5.2.
Problem was CBX Flexible CountDown by WPBoxr.
I have noticed similar problem too which is ussual after WP 4.5.2 updates.
I can’t upload new media files into Media library. I mean WP allows me to upload the file, but the actual image is not there.
I have noticed that old and new files are stored in the different location.

Strange. I had to disable woocommerce plugin to make the media library work. WP 4.5.2. Woocommerce 2.6.2 everything updated.
After disabling all the plugins and enabling them one by one, I discovered SEO Ultimate was causing the problem.
