Sorry about that. Make doesn’t officially support this plugin. If you switch themes does the issue persist for you?
Arrrgggh. It was working great. When I tried it on a dummy site with the same plugin’s but 2014 theme it works fine.
It isn’t throwing any errors in console, it is just like there is an empty div over the form starting at the title field not allowing the user to access it.
And a get a weird grey box.
This is a private site but I could set up a user if anybody form Make was willing to check it out.
I can’t switch themes right now… rebuilding in another theme would be super hard. Is there any way to get a copy of the older Make theme.
I just need to get by until the 30th when the event the site supports is over.
I will continue to troubleshoot.

Got an “error” via console:
Consider using ‘dppx’ units, as in CSS ‘dpi’ means dots-per-CSS-inch, not dots-per-physical-inch, so does not correspond to the actual ‘dpi’ of a screen. In media query expression: print, (-webkit-min-device-pixel-ratio: 1.25), (min-resolution: 120dpi)

Unfortunately we’re not able to troubleshoot third party plugins, even in our customer forums. We recommend the fine folks at Codeable if you’re having trouble with development.
I can certainly add a request that we add support for this plugin in future releases, but I couldn’t tell you when or if it would be developed.
