Sorry to hear you are having this issue.
Could you check the status in Forminator > Submissions?
To the contact submit to Mailchimp your fields need to match the format, for example, if you use an email field in MailChimp int the Forminator must be an email field, same for other fields in the mapping.

Thanks for getting back to me so quick!
ive assigned and created new merge fields. However, in forminators assign fields there are fields that arnt present on the form. Also some of the fields which require assigning in the assign fields, arnt showing as a merge field even though they are in my Mailchimp. see links to screenshots

Sorry for the delay on this.
I was able to replicate the issue on your form, for the first moment I see you are using the GDPR mapped too, the Forminator uses checkbox while you are using the radio button in Mailchimp, however, I left only First Name and Last name fields and made no difference.
I pinged our developers to verify it, we will let you know once hearing from the team.

Let me follow up here, our developer confirmed the bug, the status isn’t showing on the Forminator dashboard, we reported this and we will be working on a fix.
If you remove the GDPR field from mapping letting only the First Name and Last Name then submit the Form is the data showing on MailChimp?
