Mail Fields Not Populating when Using Safari

I’m using a multi-part form for a job application. The content from the mail fields does not come through in the email when the form is completed using Safari. It works fine when the form is completed using Chrome.

I believe the form was working correctly in both browsers before it was moved live.

Any ideas on how to resolve this issue?
