Looking for a plugin

Hi all,

I am looking for a wordpress plugin that would act similar to a complaints form, can anyone advise on such a plugin that could do this?

I would need it to have a form to fill in and also for it show an area of all the complaints entered. Also any reporting that could be done would be great.
