Is there a way to create a login page within WordPress?

I have seen plenty of posts on how to modify the current login page but I am wanting to create a page within WordPress that has a way to login to the site. here is a mockup of what I want it to look like adn not look like.

Hi Houston,

You could create a custom page template to your theme that includes a call to wp_login_form(): http://codex.wordpress.org/Function_Reference/wp_login_form

You would then include styling specific to this form in your theme stylesheet.
