Hi do you have a cache plugin installed? If you do, have you excluded the login page from the captcha plugin? Some have reported issues with the captcha plugins.

Thanks for your reply. I do not have a cache plugin installed but I’m using godaddy managed wordpress hosting and they say they use Varnish for front-end caching. Could this be effecting the captcha functionality?
Do I have to tell godaddy to exclude the login page from caching?
Fyi, They do have a way to flush the cache by appending ?nocache=1? onto http://website.com/wp-admin/. When I do this and then try to login with the captcha, it still is not working.

Captchas do not work.
The Login Captcha affects the BuddyPress Login.
The Comment Captcha can be skipped (ie Not required).
I recommend adding Google reCaptcha as an alternative in Plugin Options.
(@smac2), yes ask GoDaddy to exclude your login page from cache. Then carry out a test.
(@livingflame), I am using this plugin on many sites and the login captcha does work for me. In most cases the issue reported by many here in the forum was caused by a cache plugin either in the website or the server.
In regards to Googles reCAPTCHA option the plugin developers will investigate further your request.

Your login Captcha has problem with BuddyPress Login Form.
When I enable Login Captcha, my BP Login Form does not work. Nobody can login.

So, the problem:
— Captcha does Not work in Blog / Comments. You can comment by skipping the captcha.
— Captcha does Not work with Buddypress, Register Form.
— Captcha does Not work in Login Form. You can comment by skipping the captcha.
— Captcha does Not work with Buddypress Custom Login Form. For example, if this is: Menu / My Account / Login. Because the captcha not appear in this custom form.
— Captcha does Not work with: I forgot my password.
Please, CHECK it. And, if you can, add Google reCaptcha in Options.
Does WP Super Cache affect this plugin?
