I cannot tell for certain, but you might have installed a really old version of the Form add-on. What version is it?

Version 1.0.2 is from Feb 2013, the current version is 3.1. Version 1.0.2 is not compatible with recent versions of WordPress. You’ll need to update the Form add-on.

Do I just buy the latest form plugin ?
Or will some of the other plugins included in my old extension pro pack no longer work correctly and also need updating?
If I decide to update my extension pro pack , do I buy that , or just purchase a new license key?

You can log into your account, renew the license, and then download the latest version to install.
