List Checkboxes only showing as dots

I want a list with checkboxes on my form. I’ve created it OK, but when I look at it, only dots show instead of an input box. This happens on radio buttons too, but I want clients to be able to select 1 or more items.

You have to use checkboxes, not radio buttons.
As I said in my post, it is happening BOTH on checkboxes and on radio buttons. I tried both options and both gave the SAME result.
That’s, indeed, odd. Please delete that field and reconstruct it by clicking on “Checkbox” under the “Template Fields” panel. Once you are done with that, let me know and I’ll take a look at the form on your website to see what is going on.

OK! I just took a look and I can select more than one option for the “Please tick all the symptoms you have experienced” field. It works as expected.

I am still seeing only dots, no boxes, and when I click on the dots (just in case) nothing happens.
Must be some incompatibility with something or other, I’ll find another plugin, thanks anyway.
If you saw my screenshot, you know that the checkboxes have a box, not a dot. If you view the page while logged out, it should look just like that. If you are logged in, you might have to clear the cache.
By the way, what version of Ninja Forms are you using? What version of WordPress are you using?
Cleared cache, still the same.
Just upgraded WP from 4.2.2 to 4.5, but it looks the same in both.

However, both my client and I prefer the other’s appearance. Thank you for your help anyway. I’ll mark this as resolved.
Yeah, sometimes it is the theme’s fault which is why you can override how the theme renders objects by using CSS.
