Link portfolio item / Client

I’ve got an website with the following pages:
– ClientPage with a list of client logos
– WorkPage with a list of all work
– Single work-page

I want to be able to select an client when I create some new work so that the logo on the ClientPage automaticly links to the right Single workpage.

How can I do this? Link different custom post types? Does anyone has an idea?
