Yes, I have repeatedly logged out. No effect.
I should add that mine is a Go Daddy “managed” WordPress account/blog. But I have had no issues due to it being “managed”. I think that had more to do with it being bundled to include WordPress, a domain and hosting.

@meandthee, there’s nothing wrong in your comment template.
All fields are in place. Logged-out users can see all the text fields, logged-in users like you can only see the comment box, obviously.
Who is logged-in doesn’t need to insert nothing more than the comment.
So, the problem is my lack of WP experience. Got it. Thank you.
I logged out and went to the blog and it was perfect.
Now, how can I add those typical boxes below Comments for the author to check regarding “Notify”? Or should that be in another thread.
And then another problem I’m having activating Jetpack. Should that also be in another thread?
@meandthee, I don’t know Jetpack closely, but I think that comment notifications is already part of the plugin. If it’s not, you will need to install another plugin for this, and yes for another topic you need to open a new thread, in this case in the “Plugins and Hacks” section.
Before doing so, read the plugin’s instructions.

I’m new (duh)so I may be posting this in the wrong place. I’m using the Twenty Ten theme. And my blog is fairly well along. But no matter what I do, or check/uncheck, the only thing under “Leave a Reply” is the “Comments” text box. No “Name” or “Email” boxes above Comments. This is happening with all posts and all pages.
I’ve read that these things should be there “by default” but they are not. I’m assuming I’m doing something wrong. Just don’t know what.
