Issues with new update

Hello there,

I updated my plugin and we are having issues with it.

1. The emails sent by the plugin no longer send the {{result}}

2. If you look at this example page http://www.lenomedia.co.za/start-a-project/hourly-job-order/ you will see that the radio buttons are not displaying correctly. The “email” and “name” text fields are different than the other fields and the “comment ” box is also looking weird.
