Mike, do you have any PayPal integration yet? Any suggestions on adding it if you don’t offer it yet?

I added PayPal integration to a form I created by using a function. You should be able to gather all of the information you need to implement it from my thread on this. http://wordpress.org/support/topic/paypal-integration-5?replies=18
