Instant Email Notification doesn't send all fields

Hello,
I purchased your upgrade to have the instant notification when a user opts-in to my campaign.In the fields I have name, email, phone number, and due date (of child’s birth). I would like to have all of this information in the notification email. Otherwise just the email is useless for what I need.
Please advice as I paid the yearly price and if I can’t receive all the info, then the upgrade would be pointless.
