Inserting only one image when I select multiple | WordPress.org

When I go to the gallery tab and select the images that I’ve uploaded and then hit “Insert selected images”, it only inserts the first image into the post.

unable to produce it in my local environment, wp3.4 or 3.5

please check the FAQ for known conflicting plugins, if possible, disable all plugins and tried it again.
