Insert Form Returns Error

    So the insert form works well, the record does get inserted into the sql but returns an error

    Couldn’t perform query (1400): SELECT FROM Record123 WHERE Number=’5′ AND NameUser=’Richard Test’ AND Location=’Alabama’ ORDER BY DESC LIMIT 1 – Unknown column ” in ‘field list’.

    Any ideas regarding what is going on
