We have investigate the problem.
The problem is caused by LiteSpeed cache plugin because you combine the js. If you have deactivated the plugin, the problem will be solved.
Also, if you can disable the plugin only on the forms pages only, that would solve the problem.
We will try to have more investigation for this to let Quill Forms ignore the js combining done by LiteSpeed cache.
