Importing member fields

I am trying to import 1200 members into word press.
How do I format my CSV file to incorporate the following:
User ID
USER ACCESS:
Membership Level
Start and End Date
StatusUSER DETAILS:
Address infoI understand ow to import the basic info, but the above fields are a challenge for me. Please advise
