There is no amphtml version for that page available, as I see no link in the page source. I see that you have a custom page template selected for that page. When you do this, then you have to explicitly enable AMP for that specific page as well. You can find the toggle for this in the publish metabox for the page.
OK, that looks right. I’m not sure why it is not available for the page then. Could it be that you have a amp_is_enabled filter in your code which is disabling AMP?
My only other suggestion now is to try disabling other plugins to see if one of them are the cause.
