I don't like this plugin

Hello ks23210, Please consider reading the help section of the plugin, or the plugin’s documentation page on this site. That way, you will be able to freate a form. You don’t necessarily have to create a custom form, just use the shortcode for the default for by adding [mpmf] on your page or post, and you are done, its that much easy.
