There is a known bug in the plugin. Since in WordPress multi-site users of subsites are given limited privileges, any HTML form content gets stripped down.
We are releasing an update mid next week with a fix. If you cannot wait until then do let us know and we will try to send you beta version of our new version soon.

If you’re in a real hurry then you can always head on over to our GitHub repository and download the development version of the plugin there.

This was actually fixed in last weeks update, version 3.1 (which was released on January 26).

Hi Danny – I am still getting the same issue. I’m waiting for the multi-site super-admins to clarify which version they installed. It was installed on Friday (29 Jan), so I am guessing it was version 3.1, but I will wait to hear what they say.
