How to set question in Set?

    Hello Udit,
    After search over plugin directory I found your plugin full filled my requirement.
    Already create Sets and Subset when I click ‘add question’ or ‘manage question’ i will show ‘500 – Internal server error’. Please reply me to fix the same for me.

    I have use latest version of WP 4.4.1
