User documentation is available at http://www.kevinsspace.ca/testimonial-basics-wordpress-plugin/
You can only put one input form on a page at this time, if that is what you are asking.
So if you have five groups then you will have to setup the input forms on separate pages. I will work on multiple input forms for a future update.

Hi Kevin, I did check the documentation, you mention groups several times:
– New – You can collect testimonials in a predefined group
– Testimonials can be grouped for display
– New – The group name for the testimonial can be included as a title
But in Testimonial Basics Options, I can’t find a way to create new groups (I don’t use the Widget).
I would like to put a form on my ‘Books’ section (page) for readers to send their testimonials on Books and then show only those ‘Books testimonials’ on that same page; same for ‘Games’ and ‘Movies’. So I would like to create those 3 groups but I have no idea where to create them.
Sorry if I missed something but even after reading your documentation (which is very useful for other options I was looking for), I couldn’t find a hint on how to do that…
Thank you!
At this point each form would have to be on a separate page for each Group that you want.
Use the shortcode input as per the user docs.
ie [katb_input_testimonials group=”Group Name”]
For existing testimonials edit them and add the Group Name you want.
To display from a group
[katb_testimonial group=”Group Name” number=”all” by=”random” id=”” rotate=”no”]
