How do you edit Theme Contact Form

Hi, Need help… I need to add a few more fields to our contact form in the Weblizar Incredible Premium Theme. Also, when receiving the email I need to have the “subject line” say that’ it’s “website names’ completed contact form” since I manage several different websites for a lot of different clients.
