how can show search bar

Hello Support,

I have installed your plugin and it’s showing all the locations which i have intered.I have called it by post typle in my custom template page.

But am not able to add search bar as i m seeing the seach bar below

I have checked the check box in admin panel from search bar.

Please suggest how an i call this serchbar.
