Guestbook page entry formatting incorrect

A new entry by created by myself (logged-in) self-checks and displays perfectly OK, for example:

[Name] from [City] wrote on [Date]:
[Entry text…]

However, a normal checked entry (from a non logged-in person) displays the Name and Author in 2 separate paragraphs above the entry, like this:

[Name]

from [City]

wrote on [Date]
[Entry text…]

This looks very messy and cannot fathom out why this is happening.
I tried temporarily removing all my Custom CSS but this makes no difference.
Any ideas?
