Forms that keep data (replacing Excel shared sheets)

Hi,

I’m looking for a solution to replace the traditional shared Excel document. The document is just used to record “yes/no” answers to content, and it has no formulas.

I want it to look more “neat” too- not just something like wpData tables.

I need a plugin like Form builder or Gravity forms that actually keeps the data there (for logged in users).

Does anything exist to do this?
